Florida healthcare facilities, commercial properties, schools, and multi-family buildings are required to maintain fire-rated door assemblies in accordance with NFPA 80. These inspections are enforced through local Authorities Having Jurisdiction (AHJ), insurance carriers, and — in healthcare — through Centers for Medicare & Medicaid Services (CMS) under NFPA 101.

More than 80% of swinging fire doors fail inspection nationally. Most failures are preventable.

If your facility operates in Florida, understanding the 13-point NFPA 80 inspection criteria is essential.

The 13 Required NFPA 80 Inspection Items

NFPA 80 requires both sides of swinging fire door assemblies to be inspected and tested annually.

1. Labels Must Be Legible

Door and frame labels must be visible and not painted over.

2. No Surface Holes or Breaks

No unapproved field drilling or damage.

3. Glazing Components Must Be Secure

Fire-rated glazing must be properly labeled and installed per listing.

4. All Components Secure and Functional

Door, frame, hinges, hardware, threshold — aligned and operational.

5. No Missing or Broken Parts

Missing screws or hardware = automatic deficiency.

6. Clearances Within Code

Perimeter gaps must meet NFPA tolerance limits.

7. Self-Closing Device Operational

Door must fully close and latch from fully open position.

8. Coordinator Operation (Paired Doors)

Inactive leaf must close before active leaf.

9. Positive Latching

Door must latch automatically when closed.

10. No Interfering Auxiliary Hardware

Prohibited items include:

  • Barrel bolts

  • Dead bolts

  • Barricade devices

  • Kick-down holders

11. No Field Modifications Voiding Label

Unapproved hardware or cutting voids compliance.

12. Gasketing and Edge Seals Present

Where required, must be intact.

13. Signage Limits

Signage must:

  • Cover less than 5% of door face

  • Be attached with adhesive only

  • Not be installed on glazing

(Items 1 and 13 were added in the 2013 edition.)

Florida-Specific Compliance Considerations

In Florida, enforcement may involve:

  • Local fire marshal

  • Healthcare accreditation surveys

  • Insurance underwriting audits

  • Risk management reviews

Facilities must retain inspection reports for a minimum of 3 years and produce them upon request.
